Important: Complete your own answers before looking at or entering teachers responses. It is very important that your responses be completely independent. If you look, even glance, at the teacher's responses before you enter your own this will bias your own responses and will invalidate the test. Click here to  print the Teacher Test Form When the form appears in a separate window, make sure your printer is turned on then click on Print this Form. Ask your child's teacher to complete the form and return it to you. It should take the teacher less than 5 minutes to check the appropriate boxes. You may want to give them a stamped self-addressed envelope to make it easy for them to return it quickly. When you get the form back, immediately fold it in half so you do not inadvertently see the answers.

Log into the online test at http://tinyurl.com/deze8y and enter your answers. Only now, unfold the teacher's test form and enter them. Follow the on screen instructions.

The webserver scores and interprets the tests in batches every half hour, so it may be up to half an hour before your report is run and emailed to you. To make sure the report is not sent to your spam folder, put @adhdtreatment.org on the white list of your spam filter.
